Terms and Conditions for Booking Services with Me to You, PLLC

By booking an appointment with Me to You, PLLC, you agree to the following terms:

1. Nature of Services

Services provided are primary care visits, wellness checks, or telehealth consultations by a licensed Family Nurse Practitioner.

2. No Emergency Care

We do not provide emergency medical services. For emergencies, dial 911.

3. Eligibility

You must be at least 18 years old or have legal guardian consent for pediatric care.

4. Appointment Cancellations

Please cancel or reschedule at least 24 hours in advance. Cancellation fees may apply, and repeated no-shows will limit future booking privileges.

5. Telehealth Consent

By selecting a telehealth visit, you consent to receive care via secure video or phone. You are responsible for ensuring a quiet, private space.

6. Data Use

Information entered into the booking system may be stored by Microsoft 365 or our EMR system and used for care coordination and communication.

7. No Patient-Provider Relationship Until Confirmed

Booking a visit does not establish a patient relationship until officially accepted and confirmed.

8. Limitation of Liability

Me to You, PLLC shall not be liable for indirect or incidental damages arising from system errors or third-party platforms.

9. Governing Law

These terms are governed by the laws of the state in which you will receive medical services.
